It's not like the one Geralt wears ingame but I guess it's due to production costs of such complex jewelry.
Nonetheless it's epicly awesome
Well actually taking the model out of the ingame files shouldn't be too hard.
Afterwards you just make a mold out of it, make a model, scan it, increase its size and resolution & co and then make a new mold for the final thing ^^.
So yeah it's possible, but rather expansive.
